The Position

This role focuses on enrolling and educating patients with chronic conditions, monitoring real-time health data, and delivering proactive care coordination to improve outcomes and reduce hospital utilization. The ideal candidate is patient-focused, self-motivated, and adaptable, with a strong ability to engage patients, collaborate with providers, and drive compliance in a fast-paced, value-based care environment.

Duties & Responsibilities

  • Enroll eligible patients into Remote Patient Monitoring (RPM) programs

  • Educate and train patients on the use of monitoring devices

  • Provide ongoing guidance on chronic condition management

  • Monitor daily patient physiological data from remote devices

  • Conduct outreach calls to encourage consistent data reporting

  • Follow up with non-compliant patients to improve adherence

  • Serve as primary point of contact for assigned patients

  • Build patient relationships to improve engagement and satisfaction

  • Analyze patient data and triage abnormal readings promptly

  • Escalate critical cases to healthcare providers as needed

  • Deliver health coaching for patients with diabetes, hypertension, and CHF

  • Document patient interactions, assessments, and care plans accurately

  • Collaborate with providers to support RPM program success

  • Address provider concerns and workflow challenges professionally

  • Adapt processes based on provider or practice needs

  • Communicate patient updates and implement care changes per provider direction

  • Act as liaison between patients and providers for informed decision-making

Supervisory Responsibilities

  • No immediate direct reports

Skills & Qualifications

  • Active LPN or RN license (required)

  • Multistate compact nursing license (NLC) (required)

  • Minimum 5 years of healthcare or clinical experience

  • Experience with chronic disease management (diabetes, hypertension, CHF)

  • Familiarity with remote patient monitoring (RPM) tools and data interpretation

  • Strong patient education and coaching abilities

  • Excellent verbal communication and interpersonal skills

  • Ability to build rapport and maintain patient engagement remotely

  • Ability to analyze patient data and identify abnormalities

  • Strong documentation and charting skills

  • Attention to detail and ability to maintain accurate records

Work Environment

  • Full-time position

  • Must currently reside in one of the following states: GA, MD, NY, PA, SC, or VA.

  • Ideal candidate has a compact nursing license 

  • Candidate with a single state license willing to get additional single state licenses would be the minimum requirements

Compensation

  • Position offers an hourly rate of $26-$28, with additional benefits

Certificates, Licenses, Registrations

  • Registered LPN or RN

  • Active compact nursing license (NLC) ​​​​

  • Only candidates with an active multistate compact nursing license (NLC) will be considered with willingness to obtain New York single state license
