Nurse Care Coordinator
Full-Time Position – East Coast

The Position
In this position, you’ll support healthcare providers by delivering high-quality, tech-enabled care to patients managing chronic conditions. Our Client is looking for someone passionate about patient education, driven to handle a high volume of triage calls, and flexible in a fast-paced, evolving healthcare environment.
Duties & Responsibilities

Enroll patients in the Remote Patient Monitoring (RPM) program and provide verbal training on device usage

Monitor daily physiologic data from patient devices and ensure consistent data capture

Conduct coaching calls and follow up with patients who are non-compliant

Build strong patient relationships to support health improvement and program engagement

Analyze patient data, triage abnormal readings, and escalate urgent cases to providers

Deliver health coaching to patients with diabetes, hypertension, or heart failure

Document patient assessments, education, and care plans in medical records

Maintain accurate and concise notes for all patient interactions

Support providers by relaying patient needs and coordinating care decisions

Identify and address provider or practice workflow challenges impacting RPM enrollment
-
Adjust workflows based on provider feedback to ensure program success

Communicate effectively with providers to implement care changes and enhance patient outcomes
Supervisory Responsibilities

Skills & Qualifications

Strong patient communication and education skills

Ability to explain and troubleshoot remote monitoring devices

Clinical data interpretation and triage decision-making

Health coaching and chronic disease management knowledge

Accurate and thorough documentation skills

Proficiency in electronic health records (EHR) systems

Work Environment

Full-time position located in the East Coast
Compensation

Position offers an hourly rate of $26-$28, with additional benefits
Certificates, Licenses, Registrations

Registered LPN, LVN or RN
-
Active compact nursing license (NLC) with willingness to obtain New York single state license
