Part-Time Position – Remote

The Position
In this position, you’ll support healthcare providers by delivering high-quality, tech-enabled care to patients managing chronic conditions. Our Client is looking for someone passionate about patient education, driven to handle a high volume of triage calls, and flexible in a fast-paced, evolving healthcare environment.
Duties & Responsibilities
Provide remote patient monitoring (RPM) support for patients enrolled in chronic care programs
Educate patients on managing chronic conditions such as diabetes, hypertension, and heart disease
Monitor and analyze daily physiologic data captured from patient monitoring devices
Prioritize and conduct timely outreach based on clinical urgency and abnormal readings
Perform triage nursing assessments over the phone to address out-of-range metrics
Escalate high-risk cases to healthcare providers for immediate medical evaluation
Document patient assessments, education, and care plans in electronic health records (EHR)
Maintain detailed, accurate, and concise notes for all patient communications
Serve as a liaison between patients and providers to support care coordination and access to medical decisions
Adjust workflows and adapt care coordination processes based on provider or practice needs
Thrive in a fast-paced, evolving healthcare environment, managing high call volumes with professionalism and empathy
Supervisory Responsibilities
No immediate direct reports
Skills & Qualifications
Active LPN, LVN, or RN license in good standing
Minimum 5-year work experience working in a practice or healthcare setting
Proven experience in care coordination, triage nursing, or chronic disease management
Proficient in using remote patient monitoring (RPM) platforms and health tracking technologies
Strong ability to analyze clinical data and identify urgent care needs
Excellent verbal and written communication skills for patient education and documentation
Skilled in electronic health record (EHR) systems and maintaining HIPAA-compliant patient notes
Demonstrated ability to handle a high volume of patient outreach and triage calls
Flexible and adaptable in a fast-changing clinical environment
Strong critical thinking and clinical judgment under pressure
Patient-centered with a compassionate and proactive approach to healthcare delivery
Ability to collaborate with multidisciplinary care teams and escalate care appropriately
Familiarity with chronic condition protocols (e.g., diabetes, hypertension, COPD)

Position offers an hourly rate of $26-$28, with additional benefits
